F16V2 Power Voltage Input Very Low

My trusty F16v2 is having an issue where the input voltage is very low (1.4 volts). 

I have tested the physical output coming from the Meanwell 12v power supply and it's 11.98 volts, so I know I am connecting the proper voltage to the f16v2.  When I use my voltage tester and touch the screw terminals that are supposed to be receiving 11.98 volts, my voltage meter shows 1.4 volts. 

Further, the status page for the F16v2 also shows only 1.4 volts input (see attached). 

Please note that the when I test the actual wires connecting to the F16v2's power input terminals, they also read 11.98 volts, so I am 100% certain 11.98 volts is going into right side power input on my F16v2.  The physical connections appear fine and in clean condition. Any ideas what might be wrong?  Thank you!

I started this hobby in 2015 using an F16v2 and a single megatree.  Now I Griswold the house each Christmas with more and more and more. I currently have five Falcon controllers controlling about 15,000 pixels.

If I understand what you said, if you measure the voltage on the wire connected to the right side of the F16, you get a proper 11.98 volts, but if you measure the screwdriver slot of the board connection, you get 1.4 volts.  First thing I would do is pull the wire out and make sure that you did not clamp the insulation rather than the wire.  Verify on both the positive and negative return connections (although the two negatives are common, so it should not affect your measurement).  If you were not clamping onto the insulation, carefully visually inspect the inside of the connection.  You are looking for anything that does not belong there.
Using LOR (mostly SuperStar) for all sequencing - using FPP only to drive P5 and P10 panels.

The voltage sensors on the F16v2's were quite inaccurate at times, I'd just ignore it.

Good point, except that his voltmeter also measured 1.4 volts at the screw terminals of the board.

Thank you all for the replies.  It turns out my power supply was fluctuating in voltage.  Basically, the power supply was bad.  I replaced it and problem solved. 

Of course I only figured this out after I replaced my v2 with a new F16v4  :-[
