Media Out issue from log

So, out of the blue, I started seeing this error in the log and sometime the entire show goes black, after I turn everything off then on, it runs.  However, when this song plays, I have a very long error list of this snippet.  I replaced the MP# file w/ an older one and still the same issue.  This happens when I play the sequence alone or in a playlist.
